Title: The Use of Megaprostheses in the Management of Primary and Secondary Bone Tumors

Abstract

The surgical management of primary and secondary bone tumors has evolved significantly over the past decades, moving from radical amputations toward limb-salvage procedures that aim to preserve function, improve quality of life, and achieve oncological control. Among the most important advancements in this field is the development and widespread use of megaprostheses — modular or custom-made endoprosthetic reconstructions designed to replace large bone segments following tumor resection. This presentation provides an up-to-date overview of the indications, surgical techniques, outcomes, and complications associated with the use of megaprostheses in the treatment of both primary bone sarcomas (such as osteosarcoma, Ewing sarcoma, and chondrosarcoma) and secondary bone tumors (metastases from breast, prostate, kidney, thyroid, or lung cancer). Special attention is given to anatomical challenges in the reconstruction of proximal femur, distal femur, proximal tibia, and pelvic resections. We will explore functional outcomes, prosthetic survival, and complication rates including infection, mechanical failure, and aseptic loosening. Data from the most recent literature, as well as our institutional experience, will be discussed. Emerging technologies such as silver-coated prostheses, 3D-printed custom implants, and integration with regenerative strategies (e.g., stem cells, bone marrow concentrate) will also be briefly addressed, highlighting the current trend toward personalized and biologically friendly reconstruction.This presentation aims to provide orthopedic oncologists, surgeons, and multidisciplinary team members with practical insights into selecting appropriate patients, optimizing surgical strategy, and managing post-operative care, with a strong emphasis on maintaining long-term function and minimizing complications.
